Sometimes, but that's more like "usually." When I was younger I didn't take it super well, but I came to embrace the idea.
Deaths/endings are important.

As a player I'd be annoyed if something janky cost my character their life, just as a DM I'll retconn a death (if we catch it before moving on) if it was due to a mistake *I* made.
