Converting pathfinder to 5e here.. How does this sound: Bad guy has an aura of hunger. Character must save each round. In Pathfinder, if you fail, you’re “fatigued”. 5e: Gain a level of exhaustion. Sound OK? PCs are easily debilitated for the day after two rounds, and conceivable dead in 6.
Multiple auras are ridiculously deadly.

Just going with poisoned, or skip conditions if you don't want poison-immunity or dwarves to have an advantage against these.
Just say fail = disadvantage on attack rolls and ability checks until they finish a rest.
