In MToF, Demon Lord Madness either arises from being near/in their lair, or by going mad within eyesight of a demon lord. What mechanic (outside of the regional effect) would cause that madness? Any effect that points to the DMG madness rules. This is saying the demon lord’s particular effects override the base table.
