This is the problem with your sex-changing elves and your non-binary changelings and your hermaphrodite kobolds. People literally sideline real nb/trans humans, believing that magical/fantasy “representation” is enough. Do better. Every major adventure we’ve published since “Curse of Strahd” contains queer characters, matter-of-factly living their lives.
D&D might be filled with magic, but no magic is required to be queer in the D&D multiverse.

Agree 100%. The problem lies not with the game, the designers or the players. The problem lies with people in the real world. D&D is not a solution to the problems of bigotry, nor is fantasy literature, but they’re often a welcome escape from real life, for however brief a time.
